CE PROGRAM RECORDS & RECORD RETENTION POLICY
Really Living Today, LLC
Purpose
Really Living Today, LLC maintains complete and accurate records for each continuing education program in order to document program development, implementation, participant attendance, continuing education credit, evaluation, and compliance with applicable continuing education provider requirements.
Program Records
For each continuing education program, Really Living Today, LLC will maintain a program record set that includes, as applicable:
• A roster identifying participants who attended and/or received continuing education credit.
• Registration records and supporting attendance-verification records.
• The names of live program presenters, documentation of their qualifications, and the content presented by each presenter.
• The program title and content description.
• Learning objectives.
• The final program agenda, including instructional time and teaching methods.
• Participant materials and educational materials used or distributed during the program.
• Participant program evaluations and related feedback.
• Promotional, advertising, and registration materials used for the program.
• Documentation of continuing education hours awarded and certificate issuance, as applicable.
• Other records reasonably necessary to demonstrate compliance with applicable continuing education requirements.
Attendance and CE Credit Records
Registration, live attendance, and post-program completion/evaluation records may be maintained as separate source records. These records will be reconciled before continuing education credit is approved and certificates are issued. Supporting platform-generated attendance reports will be retained when used to verify participation.
Record Retention
Continuing education program records will be retained for a minimum of five (5) years from the date of the program. Records may be retained longer when required by law, contractual obligations, an approving organization, or another applicable requirement.
Security and Access
Continuing education records will be maintained in a secure manner designed to protect participant information and program documentation from unauthorized access, disclosure, alteration, loss, or misuse. Access will be limited to authorized individuals with a legitimate need for the information for program administration, compliance, recordkeeping, or related business purposes.
Backup and Preservation
Really Living Today, LLC will maintain reasonable backup or redundant electronic storage practices for required continuing education records so that records can be recovered in the event of accidental loss, corruption, or other disruption.
Records will be organized in a manner that allows Really Living Today, LLC to locate and produce required documentation for an authorized audit, review, or regulatory request.
Responsibility
The Program Administrator or an authorized designee is responsible for ensuring that required continuing education records are collected, maintained, secured, and retained in accordance with this policy.
